First Steps

One of the advantages of working with our contractors is that they pay a negotiated rate into the benefit plans for you. In order to take part in the benefit plans, you must enrol in the benefit plans specific to your trade. This is the first step to access the plans provided to support the health and well-being of yourself and your immediate family.

How to Enrol

You need to complete the enrolment forms for the benefit plans that are specific to your trade. The completed and signed forms must be delivered (by mail or in person) to the Benefits Administration Office for that plan. The list below provides you with the type of benefit plan, trade and contact information.

Your responsibilities

The Trustees and the Plan Administrator provide booklets, newsletters, and online access in an attempt to ensure that you are familiar with the benefit plan coverage, conditions, and limitations.

Please note that eligibility and coverage require that you:

1) Complete a Member Information Card (MIC) and file it with the Plan Administrator when you become a Member of, or rejoin UA Local 740 or if you are dispatched for work as a temporary foreign worker or permit worker. It is very important that you complete a Member Information Card for both the Welfare Plan and the Pension Plan.

The cards provide the Plan Administrator with the personal data and beneficiary information required to administer your benefits within the guidelines of the Trust Funds.

2) Complete the Coordination of Benefits (COB) section of the MIC if you or your dependents have insurance coverage elsewhere. If your Spouse has insurance coverage for herself/himself, you and/or any dependent children, the details must be given to the Plan Administrator.

COB ensures that you and your family receive maximum reimbursement for the medical and dental expenses incurred. Failure to submit the COB information or to provide COB information on the MIC and health and dental claim forms is a serious matter that could result in loss of coverage for you and your dependents.
